This is the procedure to be used for requesting a free supervised viewing of Arts and Science faculty final examinations written on the St. George campus within the last six months. This does not include in-class exams or tests, nor final exams for courses given on other campuses.

Instructions

1. Carefully read the Examination Viewing Instructions.
2. Fill in all areas of the form and print it out. Before you can access the form, your web browser will require the Adobe Acrobat Reader plugin software. Most browsers have this software. If the link above does not work,  you can get the software and installation instructions free from the Adobe web site.
3. The form must be signed by the student whose exam is being requested.
4. Submit the form in one of the following ways:
1. By fax - Fax your form to 416-978-2487.
2. By mail - Mail your request to:

3. In person - Visit us at the Office of the Faculty Registrar (hours of operation: Monday to Friday, 8:45 to 5 (to 4:30 in July and August).
